Before you enter that code
Joining a department is not the same as filling this out on your own. Here is exactly what changes, in plain terms.
Your department's administrator will be able to see
- That a summary was submitted, and when — along with the role, department and years of service on it
- The full summary itself, including the exposures and symptoms you enter. We are not going to soften that — if you would rather your department never see it, use the personal route below instead.
What never happens, either way
- Nothing is a fitness-for-duty evaluation, and nothing is sent to an employer, insurer, pension board or licensing body
- No diagnosis, no risk score, no prediction — the summary organizes history, it does not judge it
- Your data is never sold, and exposures, symptoms and high-risk incident details are encrypted at rest
- It costs nothing, for you or your department
